You are required to complete a formal health history assessment, as well as provide an electronic payment method for our scheduling software prior to your appointment.

You must agree and consent to assessment and treatment. You acknowledge that your personal health information will be collected, and understand that all information you provide will be kept confidential unless required by law. Your medical information may be shared by the various care providers involved in your care and treatment.

R+R Therapeutics Center requires notice of cancellation 24 hours prior to your scheduled appointment. Cancelling your appointment within 24 hours will result in a 50% cancellation fee. If you are absent from your appointment without notice, you will be responsible for 100% of the cost of your treatment session.

Any client 12 years of age or under must have a parent’s or legal guardian’s written consent and the parent must be present in the room as is required by law. Any client 13-17 years of age must have a parent’s or legal guardian’s written consent, in which your attending licensed professional may require a parent or legal guardian be present in the room at their own discretion.

Your attending therapist has the right to end your treatment session due to inappropriate behavior, hate speech, discrimination, or any other reason that they deem as an unacceptable form of behavior or general indecency that will inhibit their ability to perform their duties as a licensed professional. Local authorities may be alerted in the event your actions or behavior are deemed unsafe or dangerous by R+R Therapeutics Center personnel.

Treatments may be covered by extended health care plans and it is your responsibility to confirm the exact details of your coverage.
